• Life Membership Fees 2000/-
• Society reserves all rights to accept or reject any application.
• All columns should be filled as appropriate.
• Documents to be attached with Application form
  1. Passport Size Photo (A selfie will also do)
  2. MD/MS/DNB/DO certificate / Proof of Training Certificate
  3. Valid government age proof (Adhaar Card, Driving License, Passport, Voters Card etc.)
• The membership no. will be allotted and conveyed to the applicant only after reconciliation of payment and verification of all documents submitted.
• All new members will be provisionally admitted and shall be deemed to have full member only after formal ratification by the general body of YOSI

Membership Criteria :

1. Ophthalmologists in training (Post Graduation or Long term Fellowship (>1 year))

or

2. Ophthalmologists Within 5 years of completion of training.

or

3. Ophthalmologists less than 40 years of age.
